One in three adults who visits the emergency room because of an unexpected or harmful reaction to a medication does so because they are given a drug that *already* caused them harm. Why does this happen? It’s an information sharing problem.
Researchers at UBC and SFU have created ActionADE – a software solution that enables rapid documentation and sharing of standardized adverse drug event information from hospitals to the community. Find out more about ActionADE here.
